Having made an appointment, you are invited to come 15 to 30 minutes early to your session and fill out the forms OR download them here and fill them out at your convenience prior to your first session.

For those 18 years of age and older:

• Intake Form - 18 Years and Older
• Counseling Agreement - 18 Years and Older
• Individual Questionnaire - 18 years and Older

For those under 18 years of age:

• Consent for the Counseling of a Minor
• Child Behavior Checklist
• Children’s Medical History Form
• Initial Assessment and Social History
• Initial Financial Information - Under 18
